Hazard, 26, has been in fine form under Antonio Conte.
The Belgium international has netted 17 goals and chipped in with five assists in all competitions, guiding the Blues to the Premier League title.
And according to Spanish transfer outlet Don Balon, Chelsea have a plan over Hazard's future.
The Blues have tabled a player-plus-cash deal to bring either Marco Asensio, Alvaro Morata or Lucas Vazquez to Stamford Bridge.
Don Balon say Asensio is Chelsea's preferred option in the swap deal.
However, Real president Florentino Perez told the Blues that Asensio and Vazquez are going nowhere - hinting at a switch for Morata.
Conte is a huge fan of the Spain striker, who he signed during his time at Juventus.
Morata struggled to get game time under Zinedine Zidane this season.
He started just 14 La Liga games for Real as the Spanish giants won the title.
However, Despite making minimal appearances, the 24-year-old still netted 20 goals in all competitions.
